From diagnosis to survivorship, we believe in keeping our patients informed about all elements of their treatment plan, including regulations and important notifications regarding healthcare practices, privacy rights and much more.

It is also important that we understand the medical needs, concerns and any limitations that may be associated with a patient’s condition so that we may derive and implement a successful treatment plan.

To proceed with treatment through our LifeStrength Cancer Rehabilitation and Lymphedema Care facility, you will need to review some necessary documents, including privacy and release policies, consent forms and personal information requirements, associated with our program.
